PLEASE READ THIS PAGE CAREFULLY!!!

Thanks for your interest in participating in this year's event. Since this is your first time working the clinic or have worked the clinic less than three years, you will be required to attend one of the following credentialing orientations:

Tuesday, July 31, 2007 at 5pm

Wednesday, August 1, 2007 at 9am

Monday, August 6, 2007 at 5pm

Thursday, August 9, 2007 at 9am

Tuesday, August 14, 2007 at 5pm

In preparation for the orientation, we ask that you follow the following steps:

1. Contact Sue Moore ( smoozella@yahoo.com ) to be scheduled for your shift(s).
2. Download, print, and complete the documents for your position from the one of the lists below.
3. Bring the completed documents and copies of all applicable professional licenses/certifications and any other supporting documents to one of the aforementioned orientation sessions.

If all is in order, your event credentials will be printed and issued to you at the end of the orientation session. Make sure you keep your event credentials in a safe place and bring them the day you are scheduled to work. Without your event credentials you will not be able to work the clinic and you will be required to purchase an event ticket in order to enter the event site.
